Introduzione
Le infezioni causate da agenti patogeni resistenti ai farmaci sono considerate un grave problema di salute pubblica a livello mondiale. Allo scopo di controbattere le infezioni generate da patogeni resistenti ai farmaci, e per evitare di creare nuove resistenze, negli ultimi anni  è stato dato un forte impulso  alla ricerca di nuove strategie antimicrobiche . Tra queste le molecole fotosensibili occupano un posto sempre di maggior rilievo.

Nel 2019 Alpha Strumenti ha introdotto in odontoiatria un nuovo innovativo  dispositivo medico per  applicazioni fotodinamiche : ALADENT.

Che cosa è ALADENT
E’ un gel termosensibile, coperto da brevetto internazionale, inventato per veicolare l’acido delta-aminolevulinico (5-ALA) nelle mucose.  Una delle sue principali caratteristiche consiste  nel gelificare a temperature superiori ai 28°. Grazie a questo innovativo  gel,  l’acido ( prodotto con purezza non reperibile sul mercato ordinario) può penetrare nelle membrane cellulari e agire  con la massima efficacia.

Come agisce
I tessuti bersaglio e i batteri hanno un’attività metabolica più alta rispetto ai tessuti sani circostanti, per cui accumulano, in maniera selettiva, concentrazioni di precursori e fotosensibilizzanti più elevate rispetto ai tessuti sani. Una volta terminato il processo di “accumulo”, le cellule trasformano il precursore 5 -ALA in un composto endogeno, la Protoporfirina IX (PpIX). Si tratta di un potente fotosensibilizzatore  che si concentra  nelle  cellule bersaglio. La PpIX  accumulata reagisce alla radiazione luminosa sviluppando alti livelli di specie reattive dell’ossigeno (ROS). Sono questi i veri responsabili degli effetti biologici desiderati, e cioè vaso-occlusione locale, danno cellulare e conseguente apoptosi.

  • Termosensibile: si presenta liquido ma gelifica a temperature oltre 28°.

  • Alta selettività e attività verso i biofilm.
  • Non presenta tossicità.
  • La clearance di   PpIX è rapida.
  • Il trattamento è indolore per cui non serve anestesia.
  • Provoca apoptosi con  conseguente risposta immunitaria localizzata.
  • Non necessita l’uso di costose sorgenti luminose laser.
  • Permette la rilevazione ottica tramite  fluorescenza (torcia FL-02).
